Hallucination occurs when an AI model, particularly a large language model, generates content that is factually incorrect, fabricated, or not supported by any source material. The term is borrowed from psychology because, like human hallucinations, the model "perceives" information that does not exist. These outputs are especially dangerous because they are often fluent, confident, and internally consistent, making them difficult for users to detect without independent verification.
Hallucinations arise from the statistical nature of language models. Rather than retrieving facts from a database, LLMs predict the most likely next token based on patterns learned during training. When the model encounters gaps in its knowledge or ambiguous prompts, it fills them with plausible-sounding but ungrounded content.
